Where can I find copies of the consent forms I filled out?

Check your email for the subject line "Copies of your Matter and Space pilot enrollment forms". 

Before the start of the pilot, each learner should have received an email with the subject line: "Copies of your Matter and Space pilot enrollment forms". This email includes three or four direct links to the forms that you filled out as part of your enrollment in the October 2026 pilot.

NOTE: Learners who did not opt in to receive a wearable will only have three forms. 

Please reference that email for those links, and be sure to save a copy of those forms for your records within 30 days of the end of the pilot, which is December 18, 2026. If you need assistance with accessing those forms after that date, please reach out to support@matterandspace.com for assistance.
